But some players seem to only now be discovering
If players don't already know, there is a chance that killing Diablo 4's Butcher will reward them with a mount cosmetic. This cosmetic is the Butcher's Cleaver, which should have been a phenomenal adornment for the player's horse after having eventually felled Diablo 4's scariest foe, but unfortunately it Diablo 4 Items is incredibly divisive.
The controversy that has swelled around this cosmetic relates to how small the cleaver appears while it is fashioned to the horse, making it seem like an insignificant prize that is hardly visible atop many mount cosmetic layerings. This has already been known about and discussed thoroughly, but some players seem to only now be discovering the item already in their inventory following a previous tussle with the Butcher.
Indeed, the Butcher doesn't appear often and if players are casually exploring in the early game it can be horribly daunting when he does arrive. This can result in players attempting to flee the Butcher or simply being pigeonholed and their character slain, which poses a unique threat in Diablo 4's permadeath Hardcore mode.
Players can eventually turn the tide and best the Butcher themselves, though that may take quite a while and the relief that washes over them might be too overwhelming for them to acknowledge the cosmetic drop. Therefore, players have admitted to not having noticed that the mount cosmetic was already in their inventory—let alone accidentally toss it out as junk or sell it since the portrait icon and text color for cosmetic drops arguably look unremarkable and unsuspecting.
Thankfully, any missed items also appear in the player's stash, but if players don't routinely stash anything it is unsurprising that they would neglect to check for tab notifications in the stash menu's UI. Now that players are more conscious about this occurring it could mean that drops and other potentially missed legendaries might be more on their radar. Rather, it is still possible for the Butcher to be killed and not have the mount cosmetic drop since it is not 100 percent reliable.
Such rarity could also contribute to why players might not have thought to look for it in their inventories since they may have killed the Butcher multiple times and not had a cosmetic drop, and thus they didn't think to look for one elsewhere. This raises all kinds of user experience issues that could be rectified for the player's quality of life in Diablo 4, but in the meantime it wouldn't hurt for players to check inventories and stashes to see if the cleaver has already been bestowed unto them.
