This advanced technique has helped millions of couples around the world achieve their dream of parenthood. But before beginning this journey, it’s essential to understand what the process involves.

Step 1: Initial Consultation and Fertility Evaluation

The journey begins with a detailed medical consultation. Both partners undergo various fertility tests to determine the cause of infertility and assess their overall reproductive health. These may include:

  • Pelvic ultrasound

  • Ovarian reserve testing

  • Hysterosalpingography (HSG)

Step 2: Ovarian Stimulation

Once the IVF cycle begins, the woman receives hormonal injections to stimulate the ovaries. This process encourages the development of multiple eggs instead of just one.

  • Daily hormone injections are given for about 8–12 days.

  • Monitoring via ultrasound and blood tests ensures proper follicle growth.

  • When follicles are mature, a “trigger shot” is given to prepare for egg retrieval.

Step 3: Egg Retrieval (Ovum Pick-Up)

Approximately 34–36 hours after the trigger injection, the eggs are retrieved from the ovaries in a minor surgical procedure.

  • Performed under sedation or light anesthesia

  • A thin needle is inserted through the vaginal wall under ultrasound guidance

  • The fluid from the follicles is collected and sent to the lab to isolate the eggs

This procedure is typically quick and painless, with minimal recovery time.

Step 4: Sperm Collection and Preparation

On the same day as egg retrieval, the male partner provides a semen sample. If necessary, donor sperm or surgically retrieved sperm may also be used.

  • The sample is washed and processed in the lab

  • The healthiest and most motile sperm are selected for fertilization

This step is crucial to ensure that only the best sperm are used in the fertilization process.

Step 5: Fertilization

The eggs and sperm are then combined in the lab under sterile conditions. Two methods may be used:

  • Conventional IVF: Eggs and sperm are placed together in a culture dish for natural fertilization.

  • ICSI (Intracytoplasmic Sperm Injection): A single sperm is directly injected into each egg, usually recommended for male infertility.

The fertilized eggs (embryos) are then cultured for 3–5 days.

Step 6: Embryo Monitoring and Selection

Embryologists monitor the development of embryos, observing their cell division and quality.

  • Day 3 embryos: Usually have 6–8 cells

  • Day 5 embryos (blastocysts): More advanced and have higher chances of implantation

Only the best-quality embryos are selected for transfer or freezing (cryopreservation) for future use.

Step 7: Embryo Transfer

This is the final and most anticipated step of the test tube baby procedure.

  • A selected embryo (or two) is transferred into the woman’s uterus using a thin catheter

  • The procedure is painless and does not require anesthesia

  • Patients are usually advised to rest for a short time afterward

After the transfer, the embryo may implant into the uterine lining and begin developing into a pregnancy.

Step 8: Luteal Phase Support and Waiting Period

After embryo transfer, the woman is prescribed progesterone supplements to support the uterine lining and help implantation.

This is followed by the two-week wait, a critical time when the body responds to the transferred embryo. During this period:

  • Light spotting or mild cramping may occur

  • A blood test is scheduled 12–14 days later to check for pregnancy (beta hCG test)

Optional Step: Freezing Extra Embryos

If multiple embryos were created and not all are used during the first transfer, they can be frozen (cryopreserved) for future IVF cycles. This gives couples another chance at pregnancy without repeating the full stimulation cycle.
